• Evidence of identity and date of birth (for example, a birth certificate or passport

• Health information about your child (such as allergies or illnesses they need to manage) – see below links to forms and resources that need to be completed.
  • Student Medical Plan Information
  • Student Action Plan and Medical Plan
  • MSC Specific Medical Advice
  • MSC Student Health Support Plan
• Other legal orders relating to your child and their welfare (if any)
